I did not watch it.
I once had a lead screw grab the tail of the coat I was wearing. It wound up just as fast as I could remove it. I was very lucky my sleeves were not buttoned. I now have a habit of disengaging the screw and carriage drive when not needed.

I don't recall the details, but this one was bad. My son Tan, the machinist, doesn't need the reminders.
He had a video where a CNC mill went nuts and over-speeded and chucked something like a CAT 40 tool holder across the shop. Smashed through a concrete block wall some distance away.

I have a habit of turning the lathe power off of even just to check a measurement. On my Monarch you push the clutch handle down to engage the spindle. Clutch lever at both the headstock and carriage. Depending where your standing and measuring, wouldn't take much to bump the handle down. Then add that 97% of the time I'm alone out at the shop and if I had a chance to scream for help I won't be heard....

I have 2 gear selectors on my lathe headstock. The one has a detent that keeps it in a neutral position. So even if the reversing power switch gets moved and powers the motor the spindle is in neutral. So if I ever am doing anything around the part. I wait for it to stop and then shift into neutral, then do whatever I was going to do. I often forget to put it back into gear and that I am okay with. I too am alone in the shop 99% of the time so I take care.

Tim I will often give you a hard time about a lot of things. This one, I will say, excellent job! When you are often working by yourself, take that extra step or two to make sure you are careful. The age I am, I saw a lot of the ‘transitions’ in the workplace. Starting out, workboots were the norm, glasses were optional, ear plugs/hearing protection a rarity. Heck most of the shops I wirked the first few years there were ashtrays sitting on benches or machines. I wish I had been better about wearing ear protection, to get into the habit better. I wear it as needed now, but suspect the damage is done. So, if you can develop that habit as well, it won’t be tougher to do it later when you really need it. Stay safe guys. There were two major incidents that I’m aware of up here the last week, one was a fatality. A young, 26 year old mechanic, in a man basket, killed, with a woman mechanic beside him. It was some kind of rigging failure and he was struck by the crane hook from what we’ve heard. He’s gone, and I suspect her life, and many others will be changed forever.
